By submitting this application, I certify to Johnson & Wales University (the “university”) that all information in this application and in my supporting documentation is true, correct and complete. The university does not retain all application material submitted by the applicant. I hereby authorize the university to obtain my official high school or secondary school transcripts and all college or postsecondary transcripts from the schools and institutions that I have attended. I give permission to the university to contact and share information with issuing institutions or other appropriate third parties for the purpose of verifying any documentation or information I have provided or as otherwise permitted or required by law. Any acceptance or subsequent enrollment to the university is conditioned upon the university’s right to revoke such acceptance or subsequent enrollment at any time if I provide false, misleading or incomplete information or if the university learns of any past or present misconduct by me that, in the university’s sole judgment, would affect my ability to represent and uphold the standards of the university. Any enrollment at the university is conditioned upon my payment of all financial obligations incurred by me and my compliance with and upholding of all university policies, rules and regulations. Solely for avoidance of doubt, the tuition and fees students pay the university will not be discounted, irrespective of any changes in the manner or mode of learning or changes to any activities, events or programs (including revision, postponement, or cancellation of same) at any time during the academic year; the manner or mode of learning could include on-ground learning, remote learning, or a combination of the two; it could include fewer classes or restructured classes; and it could include changes mid-semester (as just a few examples of possible changes). Please understand that the university, like other schools, is subject to multiple factors outside of its control, including effects stemming from the pandemic and applicable local, state and federal law and guidance related to the pandemic, which factors could significantly alter, limit or curtail our operations. Any enrollment at the university is also conditioned on my ability to satisfy the technical standards applicable to my chosen program or courses, available at https://catalog.jwu.edu/admissions/admissionsrequirements/technicalstandards. My signature below constitutes my express written perpetual consent for the university to conduct business electronically, including electronic submissions and deliveries, with me. I acknowledge that I may notify the university of my preference to receive items via hard copy by emailing [email protected]. I further acknowledge and certify that in order to conduct business electronically with the university, I have access to an electronic device (e.g., a computer, smartphone or tablet) with a supporting operating system that can connect to the internet, an up-to-date internet browser, an up-to-date PDF viewer, and sufficient electronic storage capacity on the electronic device to enable me to save documents for later reference or be connected to a printer that is capable of printing from the browser. My signature below constitutes my express written consent to receive communications — including through phone calls, text messages, emails, faxes, and any other means — from or on behalf of the university and its affiliates, agents, employees, representatives, and contracting partners for any university purposes at the (wireless) phone number(s) and other contact information I have provided and may provide in the future. The foregoing consent includes but is not limited to receipt of communications sent by an automatic telephone dialing system or using an artificial or prerecorded voice or other automated technologies. I agree to notify the university in writing if I change, drop, transfer, or otherwise surrender any phone number(s) or email(s) provided. I authorize the university to capture and disclose/use my image, likeness, name, and/or voice (and audio and video recordings and photographs or any other medium in which they appear) without limitation in any manner or media that is accessible to the university community, external university partners, or the general public for university purposes, including, but not limited to, educational, marketing, promotional and publicity purposes. I am aware that I may update or withdraw these consents at any time, by emailing [email protected]; however, I acknowledge that such update or withdrawal may take a reasonable amount of time to process.
For applicants who currently reside in the EEA or PRC, the following questions are optional: questions related to felony-type charges or non-felony charges involving fraud or sexual or other acts of violence and questions related to ethnicity and race. The Annual Fire Safety Report discloses information about the campus fire safety policies and procedures and fire statistics for each residence hall.
